Los Gatos Restorative and Cosmetic Dentistry is located in a five-suite building on the corner of South Bascom and White Oaks Avenue in Los Gatos, California. The office is near, and easily accessible from, highways 17 and 85. As you approach the office, you will see the green and white sign that reads "Bascom Oaks Dental Center."

Monday through Thursday: 8am–5pm
Fridays: 9am–1pm
Patient Information
Please assist Dr. HagShenas’ staff by providing the following information prior to your initial appointment:
- Your completed forms
- Your referral slip and any applicable current X-rays
- A list of your current medications
- Applicable insurance information so that we can help submit dental claims to your insurance company
Please let Dr. HagShenas and his staff know if you have a medical condition that requires attention including, but not limited to the following:
- Diabetes
- High blood pressure
- Back and neck pain
- Artificial heart valves and joints
- Rheumatic fever
- Heart murmur
Also, let Dr. HagShenas know if you are on any special medications that may require special considerations.
Financing Options
You will be presented with an estimated fee prior to the completion of your dental procedure, and a payment is requested at the time services are rendered. For your convenience, we accept the following payment options at Los Gatos Restorative and Cosmetic Dentistry:

- Outside Financing: Dr. HagShenas offers Citibank Financing and Care Credit for his patients, allowing them to make monthly payments for their dental care if needed.

Because Dr. HagShenas is committed to help you receive the maximum reimbursement to which you are entitled through your insurance, his staff will help you process your insurance claims, and provide dental X-rays, photographs, and diagnostic reports as required by your insurance company.
